How do you get started?
For many people looking to work at home in medical transcription, online courses are one of the best options. It’s much like the conditions when you’re working at home.
There are three schools that have by far the top recommendation for medical transcription training. The top one is Andrews School of Medical Transcription, the second is M-Tec and the third is CareerStep’s Gold and above programs.
Andrews and M-Tec cost 2-3 times more than CareerStep. I believe all these schools offer at the very least a payment plan so that you don’t have to come up with the money all in one shot. However, the cost is why I recommend CareerStep for most people.
These schools all have excellent reputations with employers. My own former employer, Medquist, would hire employees from these schools. Their students tend to be ready to work.
These programs take at least 6 months for most people to finish, with some taking 9 or more months. Programs are pretty much self paced, but you don’t want to take excessively long. I recommend trying to work your medical transcription course just like you would work your medical transcription job. Set regular hours, tell your family not to bother you and pretend you’re working for an employer. It’s good practice for the real thing.
There are other courses aside from these three, but these are by far the top. If you decide to research other companies, check their curriculum and job placement rates, especially with any companies they do not own. Some "schools" are training centers for less reputable employers and don't really teach you what you need to know to land a decent medical transcription job.

College Programs, Community Colleges and Vocational Schools
You can find quality medical transcription training in some areas. This is good if you would prefer to attend classes in person more so than online.
Taking a Risk and Training Yourself as a Medical Transcriptionist
I don't particularly recommend this method, even though it's the one I used when I started. Things have changed since then, and this doesn't work so well for breaking into medical transcription.
Books You'll Need as a Medical Transcriptionist
Medical transcriptionists need a lot of reference material. There's a lot to know and reports can get into some very unusual or obscure terminology.
